The cardiac MRI protocol used in tumor imaging should be specifically tailored to the mass. In our experience, it is important that the radiologist be available to review the initial images; otherwise, generic image protocols and image planes may be applied that may not optimally depict or characterize the mass. Cardiac tumors are rare but reporting of cardiac masses on noninvasive testing as computed tomography (CT) or echocardiogram is common. Cardiac masses can widely range from a normal structure of the heart, also known as “pseudo-mass,” to cardiac thrombus, to the benign and malignant primary and secondary tumors of the heart. Malignant primary cardiac tumors are rare and typically have rapid expansion, invading several cardiac structures (myocardium, cardiac chambers and pericardium).
Cardiac MR imaging is the preferred method for assessment of cardiac masses. A comprehensive cardiac MR imaging examination for a cardiac mass consists of static morphologic images using fast spin-echo sequences, including single-shot techniques, with T1 and T2 weighting and fat suppression pulses as well as dynamic imaging with cine steady-state free precession techniques.
